Hospice care is a level of care provided for patients who
have a serious, life-limiting, terminal illness. Based on the
patient’s needs, hospice care may be provided in the
hospital, the patient’s home, a nursing home or an assisted
living facility.
John T. Mather Memorial Hospital is contracted with three hospice
agencies that are accredited by the Joint Commission on Accreditation
of Healthcare Organizations. There are certified hospice nurses
available on-site to answer patient and family questions and
to arrange for the transition to hospice care at home.
Patients who are placed on the inpatient hospice program are
supported by an interdisciplinary team of hospice specialists
that includes the nurse, social worker, and spiritual counselor.
